INSTRUCTIONS: Fill out the Application and it will be sent to the Program Manager. The Program Manager will review your Application and contact you by email within 7-14 days with one of two messages: (1) we are not able to accept applications at this time but we will keep your Application on file, or (2) instructions on how you can go to the next step in the process.
SPECIAL NOTE: No one answer on this Application will necessarily disqualify an individual from being accepted. The Lazarus Project is an equal opportunity institution and opposes discrimination in any form.
Before you complete the Application, please note that the Lazarus Project is a faith-based drug rehab/Christian discipleship program and may not be for everyone.
There are 10 kinds of people we cannot help. If you are a person described in any one of the 10 statements below, it would be better for both you and the Lazarus Project if you didn't complete the Application:

THE LAZARUS PROJECT CANNOT HELP A PERSON who thinks someone else is the problem.
THE LAZARUS PROJECT CANNOT HELP A PERSON who wants a different outcome than we do.
THE LAZARUS PROJECT CANNOT HELP A PERSON that expects us to make a greater investment in them than they are willing to make in themselves.
THE LAZARUS PROJECT CANNOT HELP A PERSON who will not accept a scriptural solution to their problem.
THE LAZARUS PROJECT CANNOT HELP A PERSON who refuses to take the time to bring about change.
THE LAZARUS PROJECT CANNOT HELP A PERSON who chooses relationships that will destroy them.
THE LAZARUS PROJECT CANNOT HELP A PERSON who desires to lead others astray.
THE LAZARUS PROJECT CANNOT HELP A PERSON who wants to disobey God.
THE LAZARUS PROJECT CANNOT HELP A PERSON who refuses to use their seed
(time, energy, words, thought life, money, free will, etc.) to create the future that they desire.

Although we do not charge for the program, there is a
$500 Non-refundable Registration / Administration Fee.
This fee in no way covers the cost that this ministry incurs to house, feed, transport and do the day-to-day operations that must be done for the program to be succeed.
